    題名: 高齡化社會緊急醫療服務空間分析之研究-以桃園市為例

    摘要: 台灣自 1993 年起步入高齡化社會,並於 2018 年成為高齡社會,根據國家發展委員會推估, 2025 年臺灣將邁入超高齡社會,在人口高齡化趨勢的浪潮下,未來應更加注意高齡者的生活需求。
    高齡者身體機能隨年齡增長逐漸衰退,其各種感官知覺、骨骼、關節、肌肉平衡等機能隨之退化,因此相對於其他年齡層的民眾,對於緊急醫療服務有更高的需求。衛生福利部 2019年調查結果顯示,台灣 65 歲以上高齡者急診使用人次,佔全部年齡層之 25.88%。而高齡者急診就診率中,前三名之疾病為:第一名胸痛、腹痛、身體不適等徵狀;第二名為骨折、燒傷燙傷、顱內損傷等傷害;以及第三名為循環系統疾病,如心絞痛、急性心肌梗塞、心臟衰竭、腦血管疾病等。因此高齡者遭遇跌倒或交通事故時,若無法立即獲得緊急醫療服務,可能造成死亡或難以回復原本生活型態,因此緊急醫療服務對高齡者尤為重要。
    本研究運用潛力服務可及性模型(Potential Service Accessibility),探討桃園市高齡者的緊急醫療服務,模擬現有消防機關與急救責任醫院之可及性與可服務之高齡人口數,並依據現況模擬結果,調整現有緊急醫療服務設施。研究結果顯示,不同道路流量情境下,消防機關派遣救護車至緊急事故地點可及性較好,而從緊急事故地點至急救責任醫院可及性較差,可及性差的地區多位於復興、蘆竹、大園、觀音等偏遠地區,其中又以復興區最為嚴重。在可服務高齡人口部份,不同道路流量情境下,消防機關可服務約 90%桃園市高齡人口,然急救責任醫院於僅能服務約40-50%的高齡人口。由於急救責任醫院可及性與可服務高齡較差,本研究亦針對現有醫院設備條件,模擬調整為急救責任醫院,發現其可及性與可服務高齡人口數均獲改善,但仍有復興、蘆竹、觀音以及新屋等區之部分里無法獲得改善。
    Taiwan has been an aging society since 1993 and became aged society in 2018. According to the National Development Council, Taiwan will be a super-aged society by 2025, and under the wave of population aging, more attention should be paid to the needs of the elderly.

    The physical functions of the elderly gradually decline with age, and their sensory perception, bones, joints, and muscle balance deteriorate accordingly. According to the 2019 survey conducted by the Ministry of Health and Welfare, the number of emergency room visits by senior citizens aged 65 or older in Taiwan accounts for 25.88% of all age groups. The top three diseases among the elderly in terms of emergency room attendance are: first, chest pain, abdominal pain, and physical discomfort; secondly, fractures, burns, and intracranial injuries; and thirdly, circulatory diseases such as angina pectoris, acute myocardial infarction, heart failure, and cerebrovascular diseases. Therefore, if the elderly do not receive emergency medical services immediately after a fall or traffic accident, they may die or have difficulty returning to their original lifestyle, so emergency medical services are especially important for the elderly.

    This study used the Potential Service Accessibility (PSA) model to study the emergency medical services for the elderly in Taoyuan City, simulating the accessibility and number of elderly people served by the existing fire department and emergency hospital, and adjusting the existing emergency medical service facilities according to the simulation results. The results of the study showed that, under different road traffic conditions, the accessibility of fire department ambulances to emergency locations was better, while the accessibility from emergency locations to emergency hospitals was worse. In terms of serving the elderly population, the fire department can serve about 90% of the elderly population in Taoyuan City under different road traffic conditions, but the emergency hospital can only serve about 40-50% of the elderly population. Since the accessibility and seniority of EMS hospitals are relatively poor, this study also simulated the upgrading of existing hospitals to EMS hospitals, and found that their accessibility and seniority were improved, but some areas in Fuxing, Luzhu, Guanyin, and Xinya could not be improved.
