    題名: 性加害人個別治療師心理治療經驗之探究
    A Study on the Psychotherapy Experience of Therapists for Sexual Offenders

    摘要: 本研究目的在於了解治療師與性加害人接觸的內在歷程轉變,藉以瞭解在真實的司法場域中,治療師在個別治療經驗中的感受及對自身的影響,並了解治療師與性加害人進行工作時的心理衝擊與因應方式,以及司法體制對性加害人心理治療所造成的影響。本研究以三位心理師為研究對象,並運用半結構式的訪談各進行兩次、每次1.5~2小時的訪談,並以質性研究的分析方式將個別訪談的資料整理與呈現,最後進行綜合詮釋。因此,綜合本研究中三位研究參與者分析詮釋,結果如下:
    一、治療師與性加害人接觸的內在歷程轉變
    (一)接觸性加害人前,治療師無法避免對其負面且深具破壞力的刻板印象
    (二)治療師因看見性加害人也曾是被害者的一面,進而消退原有的懼怕感受
    (三)治療師因增加對性加害人理解的視框,打破原本對性加害人的負面標籤
    二、治療師與性加害人的個別治療經驗
    (一)與性加害人在治療關係中的正向互動與進展,為治療工作帶來希望與賦能
    (二)治療師認知到性加害人的早年受創經驗後,可能停留在分裂的衝突感受中
    (三)治療師渴望處理性加害人核心議題,性加害人則關切司法處遇與假釋評估等現實問題
    (四)治療師有時難以判斷性加害人的進展是想改變自身或僅為了假釋評估
    (五)治療師可能因長期面對性加害人高創傷的暴力故事,而選擇減少第一線工作
    (六)離開晤談室的治療師,有時恐懼自己或家人成為性加害人的攻擊對象
    (七)治療師從事司法工作的疏離感一如性加害人所處在的社會隔離狀態
    三、司法體制對性加害人心理治療的影響
    (一)強制治療有機會帶給性加害人矯正性的人際經驗,改變其生命腳本
    (二)治療師因兼具評估與治療的雙重角色,成為性加害人心理治療的兩面刃
    (三)強制治療因時間與架構限制,阻礙性加害人正向積極狀態的延續
    四、性加害人治療師的訓練及專業協助
    (一)反移情的消化有助於治療師的後設思考
    (二)治療師接受督導與個人心理治療,賦予其繼續與性加害人工作的能量
    最後,本研究針對以上研究發現進行討論,並提供未來研究者對相關議題進一步的參考方向,提出具體實務建議。
    The Purpose of this study is to probe into the transformation of therapists’ mental process during their contact with sexual offenders in order to understand the therapists’ feelings in their individual treatment experiences in actual judicial field and the impact of such feelings on therapists themselves, the mental impact on therapists during their treatment on sexual offenders and the approaches adopted by therapists to cope with such impact and also the impact of judicial system on the sexual offenders’ psychotherapy. This study selected three psychologists as research objects and made semi-structural interviews on each psychologist for two times, with each interview lasting for 1.5-2 hours. Data of each interview was collected and presented, analyzed with qualitative study method and then comprehensively interpreted. After analyzing and interpreting the three research objects, this study comes to the following findings:
    I.Transformation of Therapists’ Mental Process after their Contact with Sexual Offenders:
    i.Before contacting sexual offenders, it is inevitable for therapists to have a
    stereotype about the negative and destructive image of sexual offenders.
    ii.After knowing most of the sexual offenders’ history as victims, the
    therapists gradually relieved their original feelings of fear.
    iii.With their increased understanding of sexual offenders, the therapists broke
    the negative tag imposed on sexual offenders.
    II.Therapists’ Individual Treatment Experiences with Sexual Offenders:
    i.The positive interactions and development during treatment relationship with sexual offenders have brought hope and empowerment for treatment work.
    ii.After knowing about the trauma experienced by sexual offenders in their earlier years, therapists may stay in a state of conflicting feelings.
    iii.Therapists are eager to handle the deep inside core issue of the sexual offenders, while sexual offenders are more concerned about realistic
    problems like judicial treatment and parole assessment etc.
    iv.Sometimes it is difficult for therapists to tell whether the sexual offenders's
    improvement is to change themselve or only for parole assessment.
    v.Therapists may choose to reduce front-line work because of their long
    exposure to sexual offenders’ highly traumatic stories of violence.
    vi.After leaving the meeting room, the therapists sometimes would fear of
    becoming assault targets of sexual offenders.
    vii.The therapist had feelings of alienation due to their involvement in judicial
    work, just like sexual offenders stay in the state of social isolation.
    III.Impact of Judicial System on Sexual Offenders’ psychotherapy:
    i.Compulsory treatment may bring corrective interpersonal experiences to sexual offenders and change their life scripts.
    ii.Therapists play the roles of both assessment and treatment, which becomes
    a double-edged sword for sexual offenders’ psychotherapy.
    iii.Due to time and structure limitation, compulsory treatment impedes the sexual offenders’ progress toward positive state.
    IV.Training and Professional Assistance for Therapists of Sexual Offenders:
    i.Countertransference digestion is good for therapists’ meta-thinking.
    ii.Supervision and individual psychotherapy on therapists would make them
    empowered to continue their work with sexual offenders.
    Finally, this study will make discussions based on the above findings and provide other researchers with further reference directions and practical suggestions about related topics.
